Immediate social distancing focused on the elderly population is essential. We have modeled a three-week period of complete isolation for the elderly. The longer this period, the more we are able to delay infections into the post-July period.

Secure and Save our Elders to Stave Off Corona Virus

As the new Corona virus is spreading across the globe relentlessly, panic and confusion are also rising. In such a situation, making prudent decisions becomes difficult too. But scientific knowledge and a clear understanding of the ground realities would always show the path to salvage any crisis.

The reports available from the worst affected countries such as China, Italy, Spain etc., have very clearly established by now that although the Corona virus has infected more than 5 lakh people and killed about 25000, almost 99% of the dead are elders beyond the age of 60-65. These reports have also clearly shown that none of the control measures, including the so called lock downs, have been very successful in containing the spread and also that the healthcare systems of the richest countries in the world have not been able to provide ventilators and such other advanced care because of the sheer numbers of the severely ill patients presenting to the hospitals in a short time. Considering these facts, it would be easily obvious that if the elders can be completely isolated and prevented from catching the infection, it will be fairly easy to ward off this new menace.

The most prestigious and respected institution for public health and epidemiology, the Johns Hopkins University, has released a report on March 24, titled Covid 19 for India Updates.

The most important statement in that report reads thus:

Immediate social distancing focused on the elderly population is essential. We have modeled a three-week period of complete isolation for the elderly. The longer this period, the more we are able to delay infections into the post-July period.

The modelled estimates in the report indicate that the number of cases could be halved and the seriously ill will be lesser by 6-7 folds, such that only 150 ventilators would be needed in place of a thousand!

This model of far lesser cases and complications by excluding the elders from the wave of the infections is based on a simple fact that it’s the patients with severe illness who transmit the infection the most and for longer time, and most such patients are elderly. Therefore, by keeping the elders safe from Corona, not only they can be saved, but also the infections can be curtailed, and the burden on the hospitals, doctors, nurses and paramedical personnel can all be reduced very considerably.

How do we save our elders?

The plan may appear difficult or an overreaction. Some may even find it hard to separate their beloved ones. But desperate times call for innovative actions, howsoever hard they may be.

It’s a fact that many elders are already living on their own, away from their children who are working in different cities within or outside India, communicating through the new media. For this Corona crisis, we are not sending them anywhere far away, but to a secure and quarantined dwelling within the city or area. Can’t we do so to save the situation for everyone?

Think of this. In the event of a warning of impending floods or a cyclone or a tsunami, it’s the elders and the weaker who are shifted to the safer locations on priority, because they will not be able to save themselves from the fury, while the able bodied ones would be. The Corona epidemic is also a tsunami of sorts and this tsunami too affects the elderly, while leaving the younger ones unscathed, almost all of whom recover without any complications in 4-6 days. Therefore the need of the hour is to immediately shift the elderly to safe quarantines. As the government may not be able to do so anytime soon, it shall be our own responsibility to save our elders, and thereby, all others.

It is better to keep them in complete quarantine, rather than keeping them separately in the same household with children and grandchildren. There’s always a risk that these younger ones may catch the infection from outside and infect the elders.

How to quarantine the elders?

It is desirable to quarantine all the elders above the age of 60 (UK has issued guidelines to safeguard elders above 70), particularly those with high blood pressure, diabetes, heart disease, obesity, cancer and smoking. They must be accommodated in a separate house, with all the inhabitants staying therein for at least 2 months, with no one going out or coming in. If anyone leaves the accommodation, he/she shouldn’t return in.

Therefore the younger ones who wish to stay with the elders to take care of their food, medicines and care, should be prepared to stay in. Therefore, those who need to go for work everyday, or often, cannot take up this responsibility. But those, for example, who can work from home, can take care of the elders.

Example 1
Siblings of a family can accommodate their parents and respective in-laws in one house, and one couple, may be with their children too, can stay in. All their daily needs such as grocery, vegetables etc., must be supplied at the gate, and the inmates must get them cleaned/wiped before using.

Example 2:

In an apartment complex, the seniors, may be of different families too, can be accommodated in one wing or one floor, that must be completely quarantined, with some younger ones staying there for support. Other younger residents of that wing/floor can relocate to the ones vacated by the elders. We should be willing for such adjustments under these difficult circumstances.

Example 3:
Those senior citizens and the poor and destitute, who cannot make any such arrangements on their own, must be accommodated in hotels, hostels, charitable institutions, resting facilities at religious centres etc., that are now vacant due to the lock down. These accommodations must also have adequate staff for preparing the food and for helping the elders, and all of them must stay in strict quarantine.

It’s very important that the people living in these quarantined accommodations must adhere to the isolation norms strictly. Also, it’s important to shift the elders to such arrangements within the next week; once the community transmission gets established, expected by mid April as per ICMR statements, there are chances of the elderly getting infected and then developing the symptoms after entering the quarantine, risking everyone else.

Those young ones who remain outside may get infected with Corona, but almost all of them will recover within 4-5 days on their own, simply by staying at home, and without the need for any medicines. Once the epidemic gets over, in a few weeks, the elders can return to their homes gradually.

Let’s secure our elders in isolation, and stave off the Corona scare.
